What Is Tirzepatide? Mechanism & Research Explained

Tirzepatide is a peptide that works on two hormone pathways at once instead of just one, which is why it’s usually described as a “dual agonist.” It’s already approved as a prescription medication under two brand names, and it’s also one of the most studied peptides in metabolic research, which is why you’ll see it referenced constantly alongside newer compounds like retatrutide.

How the Dual Agonist Works

Tirzepatide activates two hormone receptors instead of one:

  • GLP-1. This hormone slows digestion, increases feelings of fullness, and helps the body release insulin when blood sugar goes up. It’s the same pathway used by earlier peptides like semaglutide.
  • GIP. This is the pathway that makes tirzepatide different from GLP-1-only peptides. GIP works alongside GLP-1 to improve how the body responds to insulin, and research suggests it also plays a role in fat metabolism. Interestingly, tirzepatide actually leans more heavily on the GIP receptor than the GLP-1 receptor — researchers think this balance is part of why it tends to produce stronger effects on blood sugar and weight than GLP-1-only options.

The working idea is that hitting both pathways together — rather than just GLP-1 — improves insulin sensitivity and appetite control more than either pathway alone. Because tirzepatide has been studied for longer and in bigger trials than newer peptides, this combination effect is fairly well understood compared to some of the newer compounds still working through Phase 2 and 3 trials.


What Research Examines

Two major trial programs have generated most of what’s known about tirzepatide:

  • The SURPASS program looked at tirzepatide in people with type 2 diabetes, and found meaningful reductions in blood sugar (HbA1c) along with weight loss, generally performing better than comparison treatments.
  • The SURMOUNT program focused on weight management. In one 72-week study, people taking the highest studied dose (15 mg) lost an average of about 20.9% of body weight, compared to about 3.1% in the placebo group. A longer 176-week study also found tirzepatide reduced the risk of progressing from prediabetes to type 2 diabetes by about 94%.
  • Ongoing research continues to look at tirzepatide’s effects on liver fat, cardiovascular risk markers, and its already-approved use for obstructive sleep apnea in people with obesity.

Like other incretin-based peptides, the most commonly reported side effects are gastrointestinal — nausea, vomiting, diarrhea, constipation, and stomach discomfort — and these tend to show up more at higher doses.

The short version: tirzepatide is the more established of the two, with years of trial data and two approved indications behind it. Retatrutide is the newer, still-experimental compound trying to build on that dual-agonist approach by adding a third pathway. Whether that third pathway meaningfully outperforms tirzepatide is still an open research question — the trials that would answer that directly, head-to-head, are limited so far.
